Patients are seen by appointment. Walk-ins are seen at the descretion and approval of the office staff. Please arrive 15-20 minutes early for your first evaluation to complete necessary paperwork. Or you may choose to complete your paperwork in advance by clicking here. |

| Cancellation Policy |
| We request a 24 hour advance notice of any appointment requiring cancellation or rescheduling. A minor fee may be assessed for any missed appointment that are not confirmed by our office. |
