Your initial consultation is free of charge.  However, we require the following forms to be completed before your evaluation. To ensure the best service, fill out these forms and fax (703-807-1183) or email (contactus@infinitetech.org) them to us prior to your visit.  If it turns out you do need a service or device, competition of these forms in advance will help us get you reimbursement more swiftly. You may always call with questions.

Insurance:

Insurance companies should not dictate what kind of orthotic and prosthetic componentry you utilize, nor should they decide when you can have it. At Infinite Technologies, we reject the "managed care" mentality, fighting for your rights whether you have private insurance, Medicare, Medicaid, no-fault, workers compensation, or any other form of health coverage.
Our staff has more than 15 years combined experience working with insurance. They monitor changes to your insurance, help protect you against coverage gaps that allow your insurer to assert pre-existing condition exclusions, and research and advise you about available insurance policies. In addition to dealing with private insurance companies, we have extensive experience working with Medicare, Medicaid, V.A. Hospitals, state governmental agencies, and foreign governments for the benefit of our clients.  We will personally handle all aspects of your insurance needs, from checking benefits and filing claims, to appeals, so that you can focus on more important things, like living your life without limitations

Below are the forms that must be filled out before your first consultation.  We suggest completing these forms ahead of time and faxing them to us (703-807-1183) prior to your visit.

AAOP - American Academy of Orthotists & Prosthetists
Founded in 1970, the "Academy" as it is known within the profession, became the first professional organization established to further the scientific, educational, and professional attainments of practitioners in the disciplines of orthotics and prosthetics.  AAOP is dedicated to promoting professionalism and advancing the standards of patient care through education, literature, research, advocacy and collaboration.

ABC- American Board for Certification in Orthotics & Prosthetics
Founded in 1948, along with the support and encouragement of the American Academy of Orthopeadic Surgeons and American Orthotic & Prosthetic Association, ABC provides the most stringent credentialing standards for practitioners and facilities delivering comprehensive orthotic and prosthetic services.

AOPA - American Orthotic & Prosthetic Association
Founded in 1917, AOPA was the first national trade association committed to providing high quality business support services and products to O&P professionals and facilities. The membership consists of over 1,900 O&P patient care facilities and suppliers that manufacture, distribute, design, fabricate, fit, and supervise the use of orthoses (orthopedic braces) and prostheses (artificial limbs).

ISPO - International Society for Prosthetics and Orthotics
An international multi-disciplinary organization comprised of persons who have a professional interest in the clinical, educational and research aspects of prosthetics, orthotics, rehabilitation engineering and related topics.

NAAOP - National Association for the Advancement of Orthotics and Prosthetics
NAAOP is an organization totally committed to addressing national legislative issues of concern which impact the delivery of high quality Orthotic and Prosthetic health care services.

NCOPE - National Commission on Orthotic-Prosthetic Education
Founded in 1972, NCOPE, became the first and only accreditation body for credentialing orthotic and prosthetic (O&P), university based education programs.  NCOPE’s primary mission and obligation is to ensure educational and residency programs meet the minimum standards of quality to prepare individuals to enter the O&P profession.

Infinite Technologies is currently the only orthotics and prosthetics practice accredited by NCOPE in Northern Virginia.

ACPOC – Association of Children’s Prosthetic and Orthotic Clinics
The ACPOC is an association of professionals who are involved in clinics providing prosthetic-orthotic care for children with limb loss or orthopaedic disabilities.

National Disability Support & Information Organizations:

ADA - American Diabetes Association
The ADA offers the latest information on diabetes and living with the disease. The mission of the ADA is to prevent and cure diabetes and to improve the lives of all people affected by diabetes.

ACA - Amputee Coalition of America
The Amputee Coalition of America (ACA) is a national, non-profit amputee consumer educational organization representing people who have experienced amputation or are born with limb differences. The ACA includes individual amputees, amputee education and support groups for amputees, professionals, family members and friends of amputees, amputation or limb loss related agencies, and organizations.

ASPIRE - Adolescent Sarcoma Patient's Intense Rehabilitation with Exercise
ASPIRE, as a not-for-profit corporation, aims to educate individuals and their families about all aspects of limb loss. ASPIRE runs self help support groups and takes groups of amputees to compete in track & field, as well as swimming events for athletes with a disability.

Child Amputees
This Site that identifies online resources that are helpful to those who are either working with child amputees or who have a developing interest in them.

Landmine Survivors Network
The Landmine Survivors Network is a non-profit organization created by American landmine survivors to help the hundreds of thousand of civilian landmine victims, and to prevent new ones from ever joining the ranks.

Helping amputees worldwide by promoting advanced research and by providing prosthetic care for individuals, who otherwise cannot afford it.

NAGA - National Amputee Golf Association
Homepage of Amputee Golfer, NAGA's official magazine and the best source of information for amputees interested in playing golf again or for the first time.

NICHCY - National Information Center for Children and Youth with Disabilities
The national information and referral center that provides information on disabilities and disability-related issues for families, educators, and other professionals. Our special focus is children and youth (birth to age 22).

Prosthetics Outreach Foundation
A non profit medical service organization that provides high quality prostheses (artificial limbs) to amputees in urgent need in developing countries and in the United States.

SuperKids
A newsletter for families and friends of children with limb differences.

Capps Kids – Plagiocephaly and Cranipsynostosis Support Site  
Capps Kids’ main objective is to raise awareness of plagiocephaly and craniosynostosis through education.  The secondary goal is to offer support to families who have a child with either condition.
